Forms, Notes and Follow-ups

The Feedback panel: filling the stage's form, adding a note, scheduling a follow-up, and what a submission does

The Feedback panel beside the patient view is where a stage's work is recorded. It holds the form the administrator attached to the stage, a note field and a follow-up time. What you submit decides whether the patient moves, waits for a scheduled time, or stays.

The panel shows beside every tab except Medical History.

What a Submission Does

The platform reads the submission in this order and stops at the first that applies:

What happens

A follow-up time, with or without answers

A task named FEEDBACK SCHEDULED is created, due at that time; the stage's open tasks are resolved and replaced by it; the card's due badge shows the follow-up and the patient moves to the bucket of that time. The patient stays in the stage and no condition is evaluated

Answers matching a form condition

The stage's tasks are resolved and the patient follows that condition's output to the next stage, where they are assigned again and get new tasks; "Feedback submitted" confirms it

Answers matching no condition

The answers are saved, the patient stays in the stage with the same tasks, and the submission appears under Previous stages with View Feedback once the patient moves later

Required questions left empty

"Please fill all the required fields" and nothing is saved, not even the note

Conditions are defined by the workflow's administrator (see Forms and Conditions). A condition matches when every answer it selects is present in your submission; the first matching condition in the stage's order wins.

Scheduling a Follow-up

Use Add Follow Up Time when the patient names a time: "call me Thursday after four".

  1. Set the date and time and press Submit, with or without filling the form.
  2. The Tasks tab shows a FEEDBACK SCHEDULED task with Follow-up time and Due Time set to that moment.
  3. The card moves to Today or Upcoming by that time, and to Overdue if the time passes without a new outcome.
  4. When you make the call, record it with Update Call Status or submit the form with the answer that moves the patient.

A follow-up never moves the patient by itself. To move them on the strength of what they said, submit the answer that matches a condition.

Notes

Notes are free text on the patient, visible to every agent under Notes on the Overview.

  • From the panel: type in Add Note and press Submit with the form; the note is saved before the answers, and "Note saved successfully" confirms it.
  • From the Overview: Add Note, type, Save. Cancel discards.

Write notes for the next agent, not for yourself: what the patient said, what they were promised, what to avoid.

After a Move

The previous stage appears under Previous stages on the Overview with its Assigned Agent and dates. View Feedback reopens the submitted answers read-only in the dialog "Submitted Feedback Form", so the next agent can read what was recorded without asking the patient again.
